Iris Program
A Flexible Long-Term Care Option
IRIS – Include, Respect, I Self-Direct – is a community-based, publicly funded program in Wisconsin that gives adults with long-term care needs the ability to design their own flexible care plans, choose where they’ll live and work and determine how they’ll be involved in the community.
We’ll provide information on how IRIS plans are set up and how the program differs from other long-term care options. Our team will help you:
- Complete a Long-Term Care Functional Screen
- Learn about the types of funding available for IRIS program participants
- Determine if you meet all of the qualifications for IRIS eligibility
- Submit your referral – your request and individual information – to the IRIS program
- Social Security
- Medicare
- Bay Lake Consortium/ACCESS
- The Marketplace
