Family Care Programs
Long-Term Care that Encourages Independent Living
Designed for people with long-term care needs, Wisconsin’s Family Care benefit is the result of a collaborative effort to provide more cost-effective choices for housing and better access to necessary services while encouraging independent living and quality of life.
Current Family Care Program
Our team will explain how the Family Care program works and assist with the application and Managed Care Organization (MCO) process. The ADRC of the Wolf River Region offers:
- Long-Term Care Functional Screens to determine the level of need and functional eligibility
- Information on Family Care as well as IRIS, Medicaid fee-for-service, private pay and other options
- Help facilitating the connection with an income maintenance agency to verify financial eligibility
- Coordination of contact with an enrollment consultant once functional and financial eligibility are determined
- Social Security
- Medicare
- Bay Lake Consortium/ACCESS
- The Marketplace
